Applications & Resources

Everything you need to complete a thorough application.

For commercial sim lounges and complex risks, the full PDF application captures the operational detail our underwriting team needs for accurate quoting. Download the application, gather the supporting documents, and submit when you're ready — or use the web intake to get started faster.

Choose Your Path

Two ways to start the conversation.

Most commercial prospects start with the web intake to open a conversation, then complete the full PDF application later. Some prefer to skip straight to the full application. Either approach works.

The Full Application

Golf Simulator Application for Insurance.

The fillable PDF application captures every operational detail our specialty carriers need to quote your operation accurately. Complete it in your PDF viewer (Adobe Acrobat, Preview, or any modern PDF reader), gather the supporting documents listed below, and submit by email.

For complex risks — multi-bay sim lounges, F&B operations, mobile companies with significant volume, instruction studios with multiple instructors — this is the most thorough path to a firm quote.

Submission Requirements

What to send with your application.

A complete submission packet helps our underwriters return a quote faster. Items marked "Required" are needed before quoting; items marked "Optional" or "If Applicable" should be included when relevant. Don't have everything? Send what you have and we'll request the rest.

01

Completed application (this PDF)

The fillable application linked above. Complete every section that applies to your operation. Use the comment fields for anything that doesn't fit.

Required
02

ACORD 125 and ACORD 126

Standard commercial application forms required by most carriers. If your existing agent has these on file from your current policy, use those. If not, your agent can prepare them or we'll provide blank versions.

Required
03

5 years of currently-valued loss runs

Loss runs from your prior carriers showing claim history. Request these from your current and recent carriers (typically 3–5 business days to receive). If you've been in business less than 5 years, send what's available.

Required
04

Current declarations pages

Dec pages from your current GL, property, and any specialty policies. These show your existing coverage, limits, and premium — useful baseline for our underwriters.

If Renewing
05

Participant waivers

The waiver or assumption-of-risk document patrons sign before using your simulators. If you require waivers, send a sample. If you don't currently use waivers, that's a conversation we should have.

If Applicable
06

Employee training, safety, and maintenance manuals

Documentation of your training program for staff, safety procedures, and equipment maintenance routines. Even informal documents help — underwriters want to see that operational discipline exists.

If Available
07

Daily maintenance checklist or logs

If you keep maintenance logs (equipment checks, screen condition, projector hours, etc.), include a recent sample. Documented maintenance is meaningful underwriting evidence.

Optional
08

Incident report form

The form your staff uses to document incidents (injuries, equipment damage, complaints). Even if you've never had a reportable incident, having the form shows operational readiness.

Optional
09

Website, brochures, and photos of activity areas

Help our underwriters visualize your operation. Photos of bay setups, F&B areas, parking, exterior signage, and any unique features. Website link if available.

Required
10

State certifications and inspections

Liquor license (if applicable), health inspection reports (if F&B is a major component), state-specific certifications for your business type, fire marshal inspections.

If Applicable
11

Ownership breakdown and resumes

Brief description of ownership (sole proprietor, LLC members, corporate structure) and the operational experience of key principals. Useful for understanding the management depth behind the operation.

Required
12

Subcontractor and lease agreements

Agreements with cleaning services, instructors operating as 1099 contractors, equipment lease agreements, premises lease (if you don't own the building). COIs from any subcontractors.

If Applicable
How to Submit

Two ways to send your packet.

Whichever route is easier for you. Most agents and brokers prefer the web intake for speed; some operations prefer the email route for control over what they send.

Option 1

Web intake (fastest)

Use our detailed commercial intake form. It captures the basics in about 10 minutes and lets you upload your application and supporting documents directly through the form.

Submissions land in our system automatically and route to underwriting the same business day.

Option 2

Email submission

Complete the PDF application, gather your supporting documents, and submit via the contact form on our website. We'll respond with the right email address to send your packet to and acknowledge receipt within one business day.

Best for prospects who already have their packet assembled and prefer to send everything at once.

Common Questions

What people ask before submitting.

A few things that come up regularly. If your question isn't here, the contact form is the fastest way to reach us.

Do I have to fill out everything in the application?

Complete every section that applies to your operation. If a section doesn't apply (no F&B, no driving range, no mini-golf), mark it N/A and move on. Don't leave required fields blank, but don't worry about sections that genuinely don't relate to your business.

What if I don't have all the supporting documents yet?

Send what you have. We'll start the underwriting conversation with whatever's available and request the rest as we go. Loss runs in particular often take 3–5 business days to receive from prior carriers, so don't wait on those if you have everything else ready.

How long does the quote process take?

For straightforward operations with a complete submission, typical turnaround is 2–3 weeks from when we have all required documents. Complex risks — multi-bay sim lounges, F&B operations, mobile companies with significant volume, or accounts with claim history — commonly run 30+ days because specialty underwriters do the work properly. Quality underwriting takes time, and we'd rather take the time to get coverage right than rush a quote that misses something. We'll communicate timing expectations once we've reviewed your packet.

Can I submit if my operation isn't open yet?

Yes. Pre-revenue and pre-opening operations are common in commercial sim and we're set up to handle them. The application has a "not yet open" option for years in business and revenue questions. Include your projected opening date and any business plan documents that would help underwriters understand the planned operation.

Do I need an agent or broker to submit, or can I submit directly?

Either works. Independent agents and brokers can submit on behalf of their clients. End-buyers can submit directly and we'll either work with you directly or, depending on your state and our licensing arrangement, refer you to a producing agent in our network.

Is my information confidential during the quote process?

Yes. Application information and supporting documents are shared only with the carriers we approach for quoting on your specific operation. We don't share, sell, or repurpose your information outside the quote process. Standard insurance industry confidentiality applies.
